Contenuto	The DRG financing system needs periodic revision, in terms of disease classification and fares, to keep up with the evolution of medical science. From the case of three inpatients considered clinically identical by the surgeon and treated by the same team in the Azienda Ospedaliera Universitaria Pisana, this article analyses the DRG for stomach, oesophagus and duodenum surgery, in oreder to demonstrate that (i) the classification system is not able to adequately describe patients facing the same surgery, but starting from different health conditions, (ii) the payement fares need to be periodically updated to keep up with the evolution of medical technology, and (iii) the DGR system does not contribute to public health cost savings.
